In the video the inflatable pig lungs simulate the lungs of a person who has been exposed to normal amounts of air pollutants and has smoked a pack of cigarettes per day for about 20 years.

You can see that tar has formed a sticky black layer inside the lungs, which restricts oxygen flow and contributes to cancers forming.
